Service Coordination

LDA's Service Coordination Department assists individuals with developmental disabilities in gaining access to the full array of services and supports available to them in the community. The process begins by listening to an individual and his or her family to determine needs and wishes in regard to:

  • Development and maintenance of social relationships
  • Exploring interests, choices and capacities
  • Identifying meaningful and productive goals and activities
  • The kinds of supports desired to achieve these goals
  • Identifying competencies and skills to be developed or enhanced
  • Making safe and growth-oriented decisions
  • Developing self advocacy skills

With assistance from the Case Manager/Service Coordinator, the individual, family and service providers work together to develop and implement an Individualized Service Plan to achieve the desired outcomes. The role of the Case Manager/Service Coordinator is to:

  • Monitor service plans to assure quality and the satisfaction of the individual and family.
  • Connect families and individuals to community organizations, resource and service providers.
  • Collect information to help families and individuals make informed decisions.
  • Help represent the concerns of families and individuals to those providing services to them.
  • Help foster independence through the use of self-advocacy skills.

CASE MANAGERS/SERVICE COORDINATORS

The individual and family select their Case Manager/Service Coordinator. LDA's staff can assist them by suggesting someone who fits the needs and wishes of the individual. The decision to connect a family and a Case Manager/Service Coordinator is made by mutual agreement. This model fosters a strong personal relationship between the individual, family and Case Manager.
