LDA's Service Coordination Department assists individuals with developmental disabilities in gaining access to the full array of services and supports available to them in the community. With assistance from the Case Manager/Service Coordinator, the individual, family and service providers work together to develop and implement an Individualized Service Plan to achieve the desired outcomes. The role of the Case Manager/Service Coordinator is to:
CASE MANAGERS/SERVICE COORDINATORS The individual and family select their Case Manager/Service Coordinator. LDA's staff can assist them by suggesting someone who fits the needs and wishes of the individual. The decision to connect a family and a Case Manager/Service Coordinator is made by mutual agreement. This model fosters a strong personal relationship between the individual, family and Case Manager. |
