Case Management
Case Management promotes the achievement of personal goals, community involvement, safe and healthy decision-making, self-determination, and citizen satisfaction.
Core functions of ongoing case management services are:

The development, implementation, maintenance, monitoring of a service plan
(including maintaining benefits and HCBS waiver eligibility)

Follow-up, linkages and referrals to services

Service documentation.
In addition to providing advocacy, case managers mentor self-advocacy skills to encourage individuals to claim independence at the level their skills support.
