In the home or for the home, we provide assistance with services needed to maintain independence in your own home      

in a community of your choosing.

It's our goal to provide assistance with the skills you need to maintain your independence and remain participating and productive members of your community.  Services are determined by you with the support and input from your family, friends, advocate, and service coordinator. 


Individuals must be eligible for the OPWDD Medicaid Waiver to be eligible for Community Habilitation programming.  Bilingual staff are available to assist Spanish speaking individuals. 

We currently have openings in our Erie and Chautauqua county programs!
